Frequently asked questions
Keeping a record of qualifying work-related injuries and illnesses on OSHA's forms. The 301 is the incident report you fill in for each incident, the 300 is the log of recordable injuries and illnesses, and the 300A is the summary you post once a year.
Most employers with more than 10 employees at any point in the calendar year. Businesses in certain low-hazard industries such as retail, service, finance, insurance and real estate are partially exempt, and any employer with 10 or fewer employees is exempt regardless of industry.
A recordable injury goes on your 300 log, which covers medical treatment beyond first aid, days away from work, job transfer or death. A reportable incident means contacting OSHA directly: work-related fatalities within 8 hours, and amputations, loss of an eye or inpatient hospitalization within 24 hours.
Post the signed 300A Summary form from February 1 to April 30 each year, somewhere employees will see it such as a break room or safety board.
Your highest-ranking company official signs it, in the bottom right corner of the 300A only. That can be the owner, an officer of the corporation, the highest-ranking company official or that person's immediate supervisor. Lancaster Safety can prepare the form, but signing it stays with you.
