The National Safety Council states a preventable-injury-related death occurs every two minutes in the United States. On average, this means there are 26 deaths and 7,190 medically consulted injuries every hour.

What Went Wrong?

Investigating a workplace incident is a process aimed at understanding why such an event took place and how it can be prevented in the future. Reviewing documents, gathering information from employees, and surveying the area where the incident took place will give a clearer view of what may have occurred during the incident.

Don’t Let the Same Accident Reoccur

Most incidents are not random events, but the result of one or more root causes. Looking closely at all incidents will result in more identification of root causes, conditions, acts, or unsafe behaviors. Once those are identified, proper controls can be implemented to successfully reduce those root causes. In this training, we will take a look at basic investigation techniques and information.
